Biography
Malik Hazley, MD, Family Medicine Physician, is an affiliated provider and member of our medical staff, available to see MCH patients at the Primary Healthcare clinic location at 52 Bypass West in Lafayette.
Dr. Hazley earned his Doctor of Medicine degree from Ross University School of Medicine and completed his Family Medicine residency at Meharry Medical College in Nashville, where he served as Chief Resident. He has spent his career caring for patients throughout Middle Tennessee and is passionate about building lasting relationships that support lifelong health and wellness.
Board-certified in Family Medicine, Dr. Hazley provides preventive care, chronic disease management, wellness services, and personalized treatment plans designed to meet the unique needs of every patient. As a local and trusted primary care physician, Dr. Hazley is dedicated to helping individuals and families in our community access comprehensive care close to home!
